There are different NHS Apps. I have finger print or face recognition for patient access. Both work well. 
 

yes you need a link from your surgery

There are a few out there that link into the NHS EMIS system. I have two on my phone - the 'NHS app' itself (there is a separate NHS Covid App for track and trace) and Patient Access. They both do pretty much exactly the same things as each other.

With the NHS app you can activate it in about five minutes by putting in your personal info and following some security stuff involving scanning your face and uploading the picture page of your passport.

The Patient Access one needs a code from your GP.

Within both apps, there is a section which shows vaccinations.

I use the NHS app on my android phone no problem, the Mrs uses it on her iPhone fine too. Are you using a VPN?

I don’t have a VPN and downloaded the NHS App about 3 weeks ago. I downloaded the NHS Covid App whilst in England last year, using a postcode from a student flat from 45 years ago. I’ve also got the Spanish, Catalan, Greek and Bulgarian ones installed. Mainly because they allow completion of entry and passenger location forms on line. The Catalan one was loaded to cover trips out of municipality, later Comarca ( county ) or during curfew hours for which you had to self certify reasons, and produce either a paper or electronic form if stopped by the mossos. Catalan plod then took a photo of the form if I was stopped.

Installed the Android version of the main NHS app last night and eventually got through the security process. As far as I can see there appears to be no more information in there than in the Patient Access program, which confirms JW's assertion that the Patient Access data is fed from this NHS database.

I'll have a go at signing up with the NHS COVID app during next couple of days.

ETA: no VPN at all used in the signing up process.

You’ll have to use an English post code to get the Covid app

There does appear to be more on the NHS App than on patient access. Here are my two dashboards. I haven’t checked to find out what the extra entries are, yet.
