The Herald went down because they had command line issues that were changed for the whole industry afterwards. They operated on the principle that unless someone tells you otherwise, you assume a required step is done (in this case the doors being closed). These days it’s the opposite. Unless you tell me, I assume it is not done (doors are still open). They literally taught us this in steam packet training years ago.
